Welcome to the DOCS Health and QTC Medical Group PHA Scheduling Portal.

We are now scheduling Periodic Health Assessments through telehealth appointments for service members.

Dates Available: March 8-30, 2025.

Please click the schedule appointment button to get started, and choose a convenient time for your assessment. Appointment times are generated based on provider availability.

Please note:

• Just a friendly reminder: If you haven’t already, please complete Part A (MHA) before your appointment.
• Once your appointment is scheduled, you will receive an email and text confirming your upcoming appointment.
• Within 24 hours of your appointment, you will receive an additional email or text reminding you of your appointment.
• At the time of your appointment, you may receive a call from a “blocked” number, please answer, it will be the provider.
• PHA appointments can take 5-15 mins to complete.
