PRIVACY ACT STATEMENT AND RELEASE

Collection of information on the HealthWorks App is authorized by the Public Readiness and Emergency Preparedness Act (PREP Act), Pub. L. No. 109-148, and the President’s Proclamation of a National Emergency Concerning the Novel Coronavirus Disease (COVID-19) Outbreak. Your information will be used for screening purposes. Providing this information is voluntary, however, failure to do so will make it impossible for us to notify you of ability to work on location. We will share this information with your employer, with a call center to communicate with you, with State Department of Health personnel or medical personnel if needed, All information that you provide will be shared with DOCS, and its affiliates.Your participation in this process and any records developed as a result of your participation that could be used to identify you are confidential and will be maintained in confidence and in accordance with HIPAA. Your heath assessment will only be retained for 60 days. The Coronavirus Screening Questionnaire is designed only to screen for symptoms of certain illnesses before a person enters the workplace. The screening and temperature testing being done in addition to the safety protocols in place cannot eliminate the risk of COVID-19 exposure.
